Five Principles to Use in the Field
- Distinguish cause from trigger. A fuse opening may trigger the stop; the cause may be damaged insulation, contamination, overload, or poor design.
- Ask why detection was late. If a bearing ran hot for weeks before seizure, the root-cause discussion should include why inspection or condition monitoring did not catch it.
- Look for system contributors. Access difficulty, poor labeling, missing spare parts, bad documentation, confusing alarms, and inadequate training can multiply downtime even when they did not create the initial fault.
- Prefer specific actions. “Train operators” is weak. “Add a two-minute jam-clear procedure with photos to station HMI and train all shifts by Friday” is measurable.
- Close the loop. A corrective action that is never verified is only an intention.
Pressure Mistakes to Avoid
- Stopping analysis at the failed component.
- Using root-cause meetings to assign blame.
- Creating ten actions with no owners.
- Failing to compare repeat incidents across machines.
A Practical Breakdown Method
- Reconstruct the timeline.
- Identify the direct failure mechanism.
- Identify contributing conditions.
- Ask why existing controls did not prevent or detect it earlier.
- Assign a small number of high-value corrective actions.
- Verify later that the actions were completed and effective.
Scenario: A motor burns out after repeated overload trips. Replacing the motor is not the end. Review shows product buildup increased conveyor load, operators were trained to reset the overload, and no current trend was monitored. Corrective actions address cleaning frequency, overload-reset expectations, and current monitoring. The root cause is broader than the burned winding.
Train This Before the Next Callout
Choose one critical machine you know and apply the ideas from this chapter while it is healthy. For post-breakdown debrief and root cause, write down the normal sequence, the key evidence you would want to preserve, and the first three checks you would make if the machine stopped. Then compare your plan with the electrical drawings, PLC diagnostics, maintenance history, and the experience of another technician. A ten-minute rehearsal in daylight can remove several minutes of confusion at night.
3 AM Rule: Do not waste a breakdown. Convert it into one change that makes the same failure less likely or easier to diagnose.
